-To address academic failure and out-of-home placements
among youth in targeted North Carolina Schools.
-This program has placed 100 nurse-social
worker teams into schools across North Carolina. These
teams will work to identify children at risk for academic
failure or out-of-home placement. By working with the
child, the family, the school and county agencies, these
teams will strive to:
- Identify the Child's and Family's Strengths
- Identify the Child's Primary Need
- Connect the Child with the appropriate services
- Improve the Child's academic, social, mental
health, or physical health outcomes
