Forms to Print Out
As you complete evidence of meeting specific competencies, you should:
- make a hard copy (printout) and electronic copy (save to a diskette) for your portfolio.
- complete a Submission Form, noting which competencies are addressed and how the competencies have been met.
- have the Submission Form initialed by the course professor (if done as a course requirement) or supervisor, as well as the cooperating teacher.
- check off the competencies met on the Student Verification Form to keep track of overall progress on the competencies.
- include a copy of the Final Rating Form in the front of your portfolio. The reviewer of your portfolio will use this sheet to evaluate your work (you should sign the statement at the bottom before submitting the portfolio).
At the final conference, the student teacher will present the portfolio, including documentation of the Advanced Competencies. The supervisor and cooperating teacher will complete the Final Rating Form; the Final Rating form will be submitted with the initial licensure application materials.
