This is what you'll see:

Click the Next button, and this screen will appear:

Be sure to click on the "I already have an account with a different service provider" option. Then click Next. You'll get another screen, asking for the name of your service provider. You can enter anything here - this is just the name that will be given to the icon that is created. I used "Bill's Duke PPP". Click on Next.
Another screen will appear, asking for phone number information. Enter the phone number you use to access the Duke Server,
then be sure to click the "Bring Up Terminal Window after Dialing" checkbox! It should look like this
below:

Click on Next. You'll now be prompted for your User Name and Password. This is actually your user ID and
password. Enter them - mine is shown below for example:

Click on Next again. An IP Address screen will appear - leave it at the default of "My Internet
Service Provider automatically assigns me one."
Click on Next again. You'll now see a screen prompting you for your DNS Server Address. Enter
152.3.250.1 in the first box, and 152.3.250.2 in the second. It should look like this:

Click on Next again.
Now you'll see a screen labeled Internet Mail. This screen is really asking if you want to use Microsoft
Exchange to do Internet Mail. I'm going to assume you plan to use it - if you don't want to, you aren't FORCED to
use it after the setup -- you can use any winsock-compliant email program you want.
Click on the "Use Internet Mail" checkbox, then enter your full email id in the "Your Email Address" box, and "acpub.duke.edu"
in the "Internet Mail Server" box. For me, it looks like this:

Click on Next.
A screen will appear asking about which Exchange profile to use. Click the NEW button to create a new profile, and
give it a name (or use the default). Click the "Use this profile when starting Exchange" checkbox. I named mine
"Internet Mail", so my screen looks like this:

Click on Next.
A screen will appear proudly proclaiming that you've been set up for the Internet! Almost done - one more thing to do....
Double-click on My Computer, then on Dial-Up Networking to go to the Dial-Up Networking folder.
Make sure your connection is the highlighted icon (the one you just created),
then click on the Connections menu item
on the menu for the folder, and select Settings. Click the Redial checkbox, and enter a
number in the "Before Giving Up Retry:" field. Leave the Prompt to Use Dial-Up Networking
option selected, as shown here:

This sets up your connection to auto-redial if the line is busy.
If you haven't rebooted yet, now's the time to do so, so that all these settings will be sure to take effect.
That's it! You're now ready to try logging on!
