Welcome to the privacy survey for nurses
This survey is designed to gather information about the perceptions of nurses in relation to the privacy and security of health care information.The survey will take approximately 15 minutes to complete.
At no time will any information be collected that could identify who you are. There are no known health risks for completing anonymous online surveys. Benefits for completing this survey are anticipated to help provide nurses with a better understanding of health care consumer privacy perceptions and practices.
The survey is being conducted as a directed research project by Duke University School of Nursing Informatics graduate students, under the supervision of Dr. Linda Goodwin (contact info provided below). Your completed anonymous survey will be stored in a database on a Duke University School of Nursing computer for an indefinite period of time, and could be re-used in the future for additional learning assignments.
Your participation is completely voluntary; you may close this web page if you do not wish to participate. No compensation is provided for survey completion. Results of the anonymous survey will be posted at this site not later than May 2005 if you would like to check back to see the final report.
